Abstract

Background: Health facility damage and limited access is reported high due to the war in Tigray. Gaining understanding into the prevalence and contributing factors of modern family planning use is essential for driving future improvements.

Objective: This study aimed to identify prevalence and factors influencing modern family planning use among married women in post-war Tigray, Ethiopia.

Method: A community-based cross-sectional study design was employed from February 10 to March 20, 2024 in South-east Tigray, Ethiopia. A total of 910 married women were included. Women were selected using multistage cluster sampling method at household level. A pre-tested interview administered structured questionnaire was used to collect data using Open Data Kit (ODK) using smart phone. Associated factors were analyzed using binary logistic regression model. The odds ratio, with a 95% confidence level was used to declare a statistically significant association at p-value of less than 0.05.

Results: A total of 910 married women were participated with 100% response rate. The prevalence of modern family planning methods utilization was 29.9%. About 9.8% women used during the war compared to 60.8% reported used before war started. Urban residents (AOR = 1.8, 95% CI: 1.27–2.51), women read and write (AOR = 2.3, 95% CI: 1.56-3.42), partner education (AOR = 2.1, 95% CI: 1.29-3.35), no awareness about family planning (AOR = 0.3, 95% CI: 0.09-0.66), and women who independently make decisions (AOR = 4.3, 95% CI: 1.29-14.42) were significantly associated with modern family planning use.

Conclusions: The modern family planning methods utilization is very low during post-war. Factors such as urban residence, education, awareness, women decision making ability about own use were factors associated with use of modern family planning methods. Improving access in far to reach areas, awareness campaigns, and empowering women in decision-making is critically needed to improve family planning utilization.
